Do you want guidance on creating the best quality of life (social, emotional, physical and cognitive wellbeing) for your loved one and yourself?
We can help!
Being a caregiver can be overwhelming. Whether you care for a spouse/partner, a parent or other friend/family member, Caregiver Connections can be a source of support and information as you go through this journey. We have two levels of assistance.
Care Consultations
We welcome all your questions and inquiries regarding caring for an aging adult. We can answer general questions about programs and services, particularly our Senior Spot Adult Day Respite Program, that are available to you as you care for a loved one at home and direct you to the right resources. This Care Consultation service is free to anyone.
Please call us at 970-445-0312 or fill out our Client Contact Form to provide us with a little information about your situation to get started.
Individual Care Planning Sessions
After an initial Care Consultation, you may need more help in developing a plan of care for your loved one. We offer Individual Caregiving Advising Sessions with an Aging Life Care Professional to provide you with customized solutions, plans and resources to specifically address your unique needs. Common needs are how to determine when you need help in the home and how to find it, resolving family conflicts around what is best for your loved one, ensuring the home is set up for safety and success, specific coping strategies and general problem-solving.
For this service, we charge $100 per session. Typically, this includes a home visit and can be completed between one and three sessions. (Financial assistance available if needed)
